Hi Bart, 
This patch was taken from OFED-3.5 (compat-rdma/patches/0025-ib_srp-Backport-to-older-kernels.patch) with an addition of the backport for scsi_target_unblock.

RHEL6.4, RHEL6.5, SLES11 SP3. 
Other OSes may be added if required.

I added these files to the linux-3.12 git repository.

You can add relevant patches under compat-rdma/linux-next-cherry-picks .
