[openib-general] ibv_dealloc_pd after create + destroy of a AV fails (the resource is busy)

I'm using svn rev: 2946 on Mellanox HCA 23108.

In user level: the following scenario fails:
allocate a PD
create AV
destroy AV
deallocate PD

The problem is that the PD deallocation fails (resource is busy). If i
remove the AV creation there isn't any problem.

I attached a small test that reproduces this issue.
