[openib-general] Kernel locking rules documentation

I've tried to write up the locking rules used by our kernel IB layer,
so that they're made explicit for people implementing new low-level
drivers and new protocols.  I'd appreciate corrections and all other
comments.  Does this seem worth sending upstream for inclusion under
Documentation/infiniband?
