[openib-general] modify QP from reset->reset or reset->error fails in mthca

According to the IB spec: 
    QP state can be modified from any state to reset
    QP state can be modified from any state to error

Modify QP from reset to reset or from reset to error fail in Mellanox HCAs.
Here if the error message from the /var/log/messages:

+++++  kernel: ib_mthca 0000:05:00.0: modify QP 0->0 returned status 0a.

i guess that the problem is that the HCA don't know that this QP is in use 
until the QP is in the init state.

roland: how do you think the mthca driver should behave in those scenarios?
