It's an older Opteron box, circa 2004-2005 or so. 

I'm trying to find an Intel box I can test with.

What are the details of the machine where you're seeing this problem?

I seem to recall some ancient Dell systems had problems with PCI-X HCAs
not reappearing on PCI after an HCA reset.

Also it might be worth checking that your BIOS is up-to-date.
