[ofa-general] ib_poll_cq err: IB_WC_LOC_QP_OP_ERR

i do rdma-read in kernel, but when i use ib_poll_cq, there is a err:
scq completion failed status IB_WC_LOC_QP_OP_ERR, anyone please tell
me where is my omission? Thx.
