Reposting because I got ahead of myself on the last email.

This series adds the local_dma_lkey concept to the core and fixes
amso, nes, and cxgb3 to use the renamed capability flag.

RFC

Question: should query_device return the local_dma_lkey?  Or is hanging
it of the ib device struct sufficient.  (I like the latter).
