[ofa-general] [PATCH] ofed/docs: some fixes in OpenSM release notes

Hi Tziporet,

Added ConnectX to list of supported devices, fixed FW versions
to match OFED release notes, and removed device IDs (no reason
for them to be there, OFED release notes doesn't have them).

Please apply OFED 1.3 docs.
