I did try everything you've mentioned. However, QLogic (now Intel) doesn't have a habit of keeping their things updated when it comes to drivers. Their last two releases supported CentOS/RHEL 6.2 and were not compiling on CentOS 6.3/6.4. Funny enough their 3rd latest release (based on 1.5.3 ofed) compiles without issues on CentOS 6.3, but not 6.4. Also I'm guessing you've tried to update switch fw, <br>hca fw and systemrom.
