[openfabrics-ewg] Rollup patch for ipath and OFED

Hi, Tziporet -

Here's a preliminary rollup patch for the ipath driver.  It includes
both updates and backport changes.

It's not complete yet, but it's a start.  Applies against OFED 1.1 tree
and builds cleanly, at least for me.
