[openib-general] struct ib_device added to ib_verbs.h

I've committed a change to ib_verbs to include struct ib_device.  I used the struct ib_device in the mthca ib_verbs.h file as the basis, but modified the function calls.  There was a second small change to make cqe an input only parameter for create/modify CQ calls, since the cqe is part of struct ib_cq.
