[openib-general] MELLANOX_HCA removed in favor of MTHCA on my branch

I've removed the MELLANOX_HCA driver from my branch as several people
have suggested, since it was broken by all the API updates anyway.

(Not posting a patch because this is really just deleting a bunch of files)
