Can you try with the very latest svn bits?  It turns out there the
readb() I was using in the FW command code path is not allowed by
Mellanox HW -- all accesses need to be 32 bits wide.  I have no idea
if it could cause this problem but in the latest tree I converted it
to a readl() just to be safe.

By the way, what's the difference between this box and the one that
was working before?  Just OpenIB SW version?
