What's this "Flash Recovery" in your lspci output? sounds like you should reburn
the hca with the latest firmware.

Are you using Arbel firmware or the "Tavor compatability" firmware?
What kind of motherboard are you using?
>From my experience you should DISABLE the "MMIO above 4GB" in your BIOS settings
(under 'PCI configuration') in order to make ARBEL-mode hca's work.
