[openib-general] IBTA IP Addressing Annex

[Kanevsky, Arkady]

incorporates fixes based on Ted and Mike's comments.
Major change is ARI suggested value length is present in byte 2
and suggested value starts at byte 4 of ARI which is align to 16-byte
boundary.
