[openib-general] iser 2.6.16 update

Following the release of 2.6.16, I have commited the 
changes in iser for 2.6.16 into the trunk and also updated 
the patches needed for using iser with it.

2.6.16 update: iscsi_iser coded ported to the changes in iscsi
and the mutex backport removed.

iscsi/iser 2.6.16 update:
- removed fwd patch iser-2.6.16.patch and linux-2.6.15-iscsi_iser.diff
- added linux-2.6.16-iscsi_iser.diff to be applied on the kernel
- added open-iscsi-1.0-485-iser.patch to be applied on open-iscsi-1.0-485 user
