Impressive debugging.

Just to be sure I understand, the problem is that mthca_arbel_fmr_unmap()
is screwing up the key by sometimes making bit 3 and bit 23 different,
and this violates what the driver promised FW by setting the bit in
the INIT_HCA command, and so corruption occurs because the FW is
getting something it didn't expect?
