Hal,
We worked this out - once Eli poked me into digging deeper I noticed that the tester was bringing up the child interfaces by hand instead of at boot time - so the options weren't getting set correctly.

Can the child interface's mode (/sys/class/net/ib0.9001/mode) be
changed (to connected) ? See Documentation/ipoib.txt
