[ofiwg] merging ABI 1.1 patch series

As just an FYI, I plan on merging the ABI update series later today or tomorrow.

I have removed a couple of patches from the series to be re-worked, but I believe all others are good to go.  Once the changes have been merged, we will need to update the providers and fabtests.  Hopefully we will be able to prototype the changes in a couple of apps, so that adjustments can be made if necessary before the 1.5 release.

There are a couple of outstanding issues that are likely to extend the API that still need patches.
