The v1.16.0 release package was missing a necessary file for one of the providers.  That has been fixed in a new v1.16.1 package, plus a couple of additional fixes were added in.

The missing file problem was a gap in the CI testing as a result of migrating away from Travis CI.  The testing has been added back to the current CI.
