[Openib-windows] contributor agreement online

Folks,

I just finished setting up the contributor agreement web page.  Please check
it out and let me know of any errors you encounter.

Note that unlike the word document version of the contributor agreement this
page is setup to be a one-time thing where people can register to be
contributors.  Maintainers will then be responsible for checking that the
source of patches is a known contributor before applying any patches (i.e.
check "from" email field against entries in the database).

The form currently doesn't ask for information about contributions, nor does
it ask for address, phone, and fax numbers.  The date of the agreement is
automatically generated when the record is stored in the database.

I worried about storing all that information.  Please let me know if this
needs to be changed.

I am in the process of creating a page where one can view all registered
contributors.  One must be registered as a contributor to view the list
since I don't want to have a non-authenticated page with people's email
addresses for the reaping.
