I've applied the patch, but had to do it all manually since the patch below
didn't apply on ipoib_driver.c.  The patch wasn't created against the OpenIB SVN
repository (there's no revision 712 in OpenIB Windows yet).

I made additional changes to the INF file to change the branding to OpenIB, too.
