Committed in revision 119.  Thanks for sending!

Note that we should transition MAD users to just set the addressing information
in the AV and let the MAD layer manage the AV.  When we add AV caching and
support for redirection, it will be far simpler to just let the MAD service in
kernel-mode do the lookup and redirection management, especially if it maintains
a cache.
