[Openib-windows] New low-level driver code

Find new low-level driver code in
svn://windows.openib.org/gen1/branches/MTHCA.
The driver is intended to support all Mellanox HCA chips and is under work
now.
The code is not operational, it is placed just for review and comments.
The short description of the status quo can be found in
MTHCA/hw/mthca/readme.txt.
