[Openib-windows] Create AV with GRH from user space

We need to make sure that this gets updated anytime the CA gets queried.  How
does the OpenIB Linux stack handle this?  Does it do an IOCTL for the lookup on
every AV created?

I'll apply this tomorrow as soon as I get a chance.
