[Openib-windows] MTHCA branch update

I uploaded the latest code to OpenIB branch 
it include :
 
    update to the mthca driver
    update to files in the core still without the patch you sent
    update of IPoIB to use regular mutex (patch was sent)
    update to ib_types.h patch will be sent today 
 
After you apply the patches I think we can move the mthca code to the
main trunk.
 
 
We now in testing phase testing. 
 
Driver Status:
 
    Driver is loaded both Tavor/Memefree
    OpenSM up
    IPoIB pass simple tests (ping)
    alts tests pass
    basic test from our regression (UC\UD\RC with multiple QPs and
multiple threads)
 
You are more then wellcome to review the code
