[ofw] RE: [PATCH] [ib-diag] ibstat: port_dump is called too many times

I've reverted the previous change to ibstat and added a change to libibumad to
give it the same behavior that OFED has.  Port data is now maintained in the
umad_ca_t port array starting at index 1, rather than index 0.  Index 0 is now
reserved until such time that someone wants to run WinOF on an IB switch.
