I was thinking more of only keeping one disconnect call that transitions the QP
to the error state.  This matches up with the way Connect/Accept transition the
QP to RTS.

On a separate note, I'm not worried about new IOCTLs, as long as it doesn't
break the existing interface.  But I don't think we need that in this case.
