[ofw] [PATCH 0/12] ND 2.0 provider

I'm not going to repost the entire series, but I made the following changes:

- Updated copyright information.
- fix 32-bit build of SRQ

The code builds for 2008 and 2008 R2 32/64-bit platforms only.  It cannot build on 2003 because FILE_SKIP_COMPLETION_PORT_ON_SUCCESS is missing.

There are still a couple more patches to add beyond these that I will add, but I will commit these changes so other patches can be applied over these.
