Out-of-date or corrupted firmware can cause this timeout/notifier.
The Connect-X HCA driver is not loading for some reason.

What does not make sense in your case is the fact you can GUI install correctly?

Do you know for a fact the HCA firmware is the latest?

Seems you have HCA drivers already installed?
Be cautioned about windows update installing Mellanox HCA drivers; this could be your problem.

Prior to install you should try http://www.openfabrics.org/downloads/Windows/etc/ibscan.bat | ibcleanup.bat
