Out-of-date or corrupted firmware can cause this timeout/notifier.
The Connect-X HCA driver is not loading for some reason.
What does not make sense in your case is the fact you can GUI install correctly?
Do you know for a fact the HCA firmware is the latest?
? pondering....
>From the install log file:
DIFXAPP: INFO: Installing devices with Id "PCI\VEN_15B3&DEV_673C&SUBSYS_002815B3&REV_B0" using INF "C:\Windows\System32\DriverStore\FileRepository\mlx4_bus.inf_amd64_neutral_053d93a28fa09302\mlx4_bus.inf".
DIFXAPP: INFO: Installation did not occur because the current driver on the device is the same or better.
DIFXAPP: INFO: No drivers installed. Drivers contained in 'C:\Windows\System32\DriverStore\FileRepository\mlx4_bus.inf_amd64_neutral_053d93a28fa09302\mlx4_bus.inf' are not better than current one's.
DIFXAPP: INFO: RETURN: DriverPackageInstallW (0x103)
DIFXAPP: INFO: The driver did not get installed on the device because the already 'existing' driver is a better match.
Seems you have HCA drivers already installed?
Be cautioned about windows update installing Mellanox HCA drivers; this could be your problem.
Prior to install you should try http://www.openfabrics.org/downloads/Windows/etc/ibscan.bat | ibcleanup.bat
