[ofw] branch\mlx4 no longer builds - missing file ibat_ex.h ?

Hi Leo,
  There seems to be a missing file iba\ibat_ex.h included from ibat_ex.cpp ?
Would you commit iba\ibat_ex.h please.

Additionally, I would like to propose that I merge the missing parts of \trunk\ to branches\mlx4 such that when we merge mlx4 to the trunk\ there are no surprises.
A nice side effect is we all can test branches\mlx4\ without disrupting the ongoing trunk\ work.
