[Users] iSER initiator flapping with OFED 1.4

Hi folks,

I am seeing some issues with iSER, a Illumos target and a linux initiator.
Basically the initiator connection starts flapping under IO load and we see
messages the system logs. [1]

If we switch back to IP mode (ib_ipoib), things work fine (although slower).

Initiator kernel: 3.2.0-60-generic
Mellanox Firmware: mlx4_0 2.9.1000
open-iscsi version: 2.0.871

Target kernel: omnios-8d266aa
Mellanox Firmware: mlx4_0 2.8.600

Obviously I intend to true-up the firmware on the target machine when I get
a service window.

Hoping that people have encountered this before...
