[openib-general] kDAPL needs less files

I would be happy to submit patches, but I am not sure that would be the
best way to do this.  I think kdapl (dat-provider) should have less .c
files, that all the functionality of a given subsystem (ia, ep, ep,
etc.) should all be in one .c file instead of lots of little files.
There are ones with only one function in them.

So, James, I don't know if you agree, but if you do, how would you like
to proceed?
