Hello Alex,
  Some good news and some not so good news.

Good news:
  IPoIB NDIS 5 version is working fine (Tx,Rx & ftp), installs & uninstalls correctly, IPoIB disable/enable OK.

  IPoIB NDIS 6 loads with Tx, Rx & ftp all working OK; with and without IPv6 support enabled.

Not so good news:

We have come full circle back to the IPoIB disable hang due to IPv6 being enabled...

In order to disable IPoIB one must 1st disable IPv6 support in IPoIB properties, reboot and only then will IPoIB disable/enable without hanging the device mgr window.
Same disable IPv6 and reboot in order to uninstall WinOF without hanging the system; during uninstall IPoIB device instances are disabled prior to uninstall.

What happened, the disable/enable was fixed just last week?
I updated to latest svn as of noon Wednesday PST.

Please advise.
